He was predeceased by: his husband Duane Leslie Cook. He is survived by: his sister Donna; his children, Michael W., Stephen M. Cook and Faith Truax (Joel); his grandchildren, Angel Polhamus, Daniel Cook and Brianna Truax; his great grandchild Dane Martin; and his siblings-in-law, Darwin Cook (Liz), Pat and Alene. He is also survived by numerous nieces and nephews.
