He was predeceased by: his parents, John Sladek and Beverly Sladek (Cunningham); and his sister-in-law Rosalie Sladek. He is survived by: his wife Lisa Sladek; his daughter Nicole Sladek LeBaron (TJ); his grandson Bryson; his sons, Scott and Dylan Sladek of Bozeman, MT; his siblings, Barbara (Paul Mossner) of Manchester, IA, James Sladek of Butte, MT and Joseph (Laural Sladek) of Charlevoix; his aunts, Janice Farmer and Sharon Woodbury of Charlevoix, MI; and his in-laws, Gary and Beverly Cantwell of Beaver Island, MI. He is also survived by several nieces and nephews.
In lieu of flowers, memorial contributions may be made to Saint Mary's Catholic Church, Charlevoix Fire Department (Search and Rescue), and Parkinson's Foundation.
