He was predeceased by: his parents, John Kozlowski and Alexandria Kozlowski; his wife Bernice Esther Patz; his son John R. "Jay" Kozlowski, Jr.; and his siblings, Mildred, Harriett, Irene, Elizabeth, Estelle, Joseph and Bernard.
He is survived by: his son James Kozlowski (Debra); his grandchildren, Brad Prince, Amy Bateson (John) and Chad Prince; his great grandchildren, Chelsi Bateson and Mariah Bateson; and his sister Joann Obergfell. He is also survived by nieces and nephews.
The family prefers contributions in memory of John be made to Wounded Warriors, Post Office Box 758516, Topeka, Kansas 66675-8516.
