He was predeceased by: his parents, John Henry and Rosa Cook (Brown); and his siblings, Mayden David (Jessie), John Henry Jr. Cook (Nadine) and Felder Cook (Claire).
He is survived by: his wife Margaret "Peggy" Cook; his children, Sheryl Makolin (Dave) of Laurium, Terry Cook (Linda) of Norway, MI, Tim Cook (Susanne) of Huntsville, TX, Dale Cook (Lisa) of Ripon, WI and Chris Cook (Aletha) of Rice Lake; his brother Clarence "Bobby" Cook (Lisa) of Hanahan, SC; and his sister-in-law Claire Cook of Goose Creek, SC.. He is also survived by 16 Grandchildren, 19 Great Grandchildren and 2 Great Great Grandchildren.
