He was predeceased by: his parents, Foster Lewis and Betty Lewis; his father-in-law Ralph Noffke (Betty); and his grandson John Thomas Smith. He is survived by: his wife Christine Lewis; his children, Angela Rushlow (Brian), Dean Lewis (Darcey), Matt Lewis (Stephani), Tim Smith (Mary) and Ed Smith (Sherri); his sisters, Linda Boot and Luann Lewis-Snyder; and his mother-in-law Theressa Noffke. He is also survived by his 13 grandchildren.
Those who wish may make memorial contributions to Faith Hospice or St. Jude Children's Research Hospital.
