He was predeceased by: his parents, Jeff Leyko and Kenda Leyko; his cousin Kristina Renae Coon; his uncle Tony Holmes; his grandparents, Ruth Ann Thrush, Robert Joseph Leyko, Gloria Jean Candy and Kenneth Blevins; and his great-grandparents, Johnson and Youngs.
He is survived by: his children, Kaylyn, Brenna and Rylan; and his siblings, Bryan Blevins (Sandy) and Carrie Patrick (AJ). He is also survived by aunts, uncles, cousins, nieces and nephews.
