He is survived by: his wife Debi Cook; his children, Debbie Fox, Scott Cook (Kelly), Lonnie Maxwell-Cook (Melissa) and Carole Kilburn (Chris); his grandchildren, Chris, Michelle, Andrew, Nathan, Justin, Kailey, Olivia, Ella and Mason; and his sister Marilyn White.
