He is survived by: his wife Bernice Hewett; his children, James, Jeffrey Hornak and Ann Marie Parker (Scott); his grandchildren, Alan, Kayla, Donavan and Malynda; and his siblings, Barb Kukulis (Ernie), Tony Hornak and Good (Kurt Park). He is also survived by nieces, nephews, and friends.
