He was predeceased by: his parents, Trefflie Forrest and Florence Forrest (Brabrant); his granddaughters, Michelle and Patricia Millward; his sisters, Dolores Cartwright, Helen Watts and Virginia Lemperle; and his stepsons, John and Kelvin Mikolowski.
He is survived by: his wife Helen Forrest; his children, Carol Millward (Chris) of Sault Ste, Steve Forrest (Sandy) of Logansport, IN and Terri Forrest of Brimley, MI; his grandchildren, Chris Millward (Debbie), Laurie Millward, Sara Gordon (Ron), Erica Balyeat (Jason Sherlund), Jason Feighner, Jenni Feighner-Fultz (Brennon) and Chris Forrest (Josie Roos); his stepchildren, Cindy Plitz, Rick Mikolowski (Kamie) and Jeff Mikolowski (Sue); and his sister Dorothy Whaley (Jack) of Goldsboro, NC. He is also survived by many step-grandchildren and great grandchildren.
