She was predeceased by: her husband Dr. Donald H. Polk; her son Gary Polk (Tammie); and her siblings, Joan Horvitz, Karl Lewis and Allen Lewis.
She is survived by: her children, Ellen Craw (David Hoke) and Kenneth Polk. She is also survived by many loving cousins and friends.
