He is survived by: his wife Judi (Smith); his children, Rick Sack (Brenda) of Rockford, Todd Sack and Deana St (Theodore); his sister Annie Clements; and his parents-in-law, Scott, Candace Smith, Debbie, John Metcalf, Tim and Michelle Smith. He is also survived by several nieces, nephews, and great-nieces.
