He is survived by: his children, John Hazewinkel of Mason, MI, Jeffrey Hazewinkel of Hamden, CT, Joe (Alane Hazewinkel) and Julie (Rhett Luedtke); his grandchildren, Hannah Hazewinkel, Jacob Hazewinkel, Jessica Jaras (Nicolas), Case Hazewinkel, Oliver Hazewinkel, Isaac Luedtke and Simon Luedtke; and his close friend Judy Nevseta.
In lieu of flowers, the family recommends memorials be given to the Music Fund at St. Peter's Lutheran Church, 408 W Bellevue St, Big Rapids, MI 49307, to honor Jack and the Praise Team band he so loved.
