He is survived by: his mother Maureen Dohm; his wife Nicole Dohm (White-Polaskey); his children, Steven Polaskey (Meredith), Brandon Polaskey (Lindsey), Nathan Dohm, Tyson Dohm, Avalon Dohm (Isaac) and Roger Dohm; his brothers, Bryan Dohm (Stacie) and Bradley Dohm; and his grandchildren, Arthur Polaskey, McKenna Polaskey and Lucy Polaskey.
