He was predeceased by: his parents, John R. Hoornstra and Helen Hoornstra (White); his wife Sharon K. Lewis; his grandson James Hoornstra; and his sister Trudy Hoornstra.
He is survived by: his children, John L. Hoornstra of Tuscola, James W. Hoornstra (Vicki) of Grayling and Susan Godde (Brian) of Ludington; his grandchildren, Monica Bayerl (Brock), John H. Hoornstra, Trudy Dukes (Tom) and Christi Godde; his great grandchildren, John W. Hoornstra, Evan and Sylvia Bayerl; and his sister Mary Johnson (Dan). He is also survived by many nieces, nephews and friends.
