He was predeceased by: his parents, Carl E. Ballard and Nadine Ballard. He is survived by: his wife Diane Perreca; his daughters, LeeAnn Golembiewski (Ken), Heide Maas (Jonathan) and Jessica Lesko (Albert); his grandchildren, Adaline, Michael, Albert V and Carl; and his siblings, James Ballard (Barb), Thomas Ballard (Gail), Candace White (Gary) and Mark Ballard (Shirley). He is also survived by many nieces, nephews.
In lieu of flowers, memorial contributions will be made at the request of the family.
