He was predeceased by: his parents, Oswald Katz and Martha Katz (Clary); and his sister-in-law Carole Katz. He is survived by: his wife Jacquie Katz; his brother Larry Katz; his children, Mark Katz (Erin) and Beth Wenner (Mike); his grandchildren, Matthew Wenner (Seetharamani), Elizabeth Folger, Page Zuk (Jordan), Rachel Katz, Meredith Katz, Elle Kinch and Sam Kinch; his great grandchildren, Wesley Folger, Chance Wenner and Zander Zuk; and his nieces, Jill Schwartz and Julie Brophy.

In lieu of flowers, the family requests donations be made In Memory of Jay Katz to support Michigan Hockey Scholarships or the Merkel Cell Research Fund — details to donate can be found below. University of Michigan Hockey, Donations will support Michigan Hockey Scholarships at the University of Michigan.
