He was predeceased by: his parents, Joseph F. Cook and Yolanda Strobel Cook; his brothers, Joseph and Bill; and his pet Bella.
He is survived by: his children, Pamela A. Maier (Jarrett) and Fred E. Cook Jr. (Holly); his grandchildren, Fred E. "Bubba" Cook III and Amber J. Duryea (Kyle); his great grandchildren, Fred E. "Bud" Cook IV and Piper J. Duryea; and his sister Joyce Vine.
