He was predeceased by: his parents, John Cook and Violet Cook (Donald); his former wife Paulette; and his children, Patrick Cook and Yvonne Cook.
He is survived by: his wife Rita Tadajewski; and his children, Denise Wekwert (Craig), Jacqueline Piwowarski (James), Lydia Cook, Jean Cook (Carol) and Tommy Cook. He is also survived by 10 grandchildren.
Interment: Long Rapids Cemetery, In lieu of flowers memorials may be made to MyMichigan Medical Center's Cancer Center.
