He was predeceased by: his father David Ellsworth. He is survived by: his mother Marlene Ellsworth (Burnham); his wife Cindy Rembowski; his children, Victor Cooke (Bethany), Kalob Ellsworth (Amanda) and Chelsea Ellsworth; his grandchildren, Jasmine, Layla and Vidar; his great grandchild Waylon; and his siblings, Michael Ellsworth (Christina), Debra Wcisel-Ellsworth, Randy Ellsworth (Marissa), Karen Ellsworth and Kathy Whittemore (Jason). He is also survived by several nieces and nephews.
