He was predeceased by: his parents, John Kolasa and Julia Kolasa; his brother David; his in-laws, Bob and Marti Pugh; and his brother-in-law Bob.
He is survived by: his wife Lynn; his sister-in-law Gerrie Jensen; his nephew Brad Pugh (Megan); his great-nephew Landon; his great-niece Lucy; his niece Stephanie Dockus (Eric); his great-nieces and great-nephews, Anna, Ryan, Tanner, Hayden, Barry and Janet Forintos; his niece Jennefer; and his nephew Anthony.
