He is survived by: his wife Tina Cook (Chappell); his children, Brandon, Katie Newhouse, Abby Cook and Joe Haas; his grandson Brayden Haas; his siblings, Larry Cook, David Cook and Mary; and his sister-in-law Bette Cook.
