He was predeceased by: his parents, Joseph Kozlowski and Gertrude Klockowski Kozlowski; and his brother-in-law Kenneth Shanklin. He is survived by: his sister Elaine F. Shanklin of Dyersburg, Tennessee; his nieces, Kim Wilson of Halls, Tennessee, Kelly Gibson of Dyersburg, Kathy Warner of Finley and Kristine Wilson of Dyersburg, Tennessee; his great-nephews, Gary Fowler (Lindsey) of Dyersburg and Cody Wilson (Lauren) of Dyersburg, Tennessee; and also, Baylee Grace Fowler, Laynee Jane Fowler and Grey Allen Fowler.
