He was predeceased by: his parents, Henry Ax and Geraldine Ax (Repo); and his wife Karen Ax (Fisher). He is survived by: his children, Dean Ax, Jeff Ax (Mary), Steven Ax (Shanna) and Michele TerAvest (Chad); his brother Eugene Ax (Phyllis); his grandchildren, David (Renae), Sarah, Rachel, Tyler, Tim, Matt and Zach; and his great grandchild Delilah.
