She was predeceased by: her parents, Herbert Ptak and Shirley Ptak. She is survived by: her daughter Andrea Kotelka (Jamie Johnson); her grandchildren, Rylan Powers and Sofie Attard; her sister Bonnie Ptak; her goddaughter Ashley Godziebiewski (Mark); her niece Kristy Brannon (late Brad); and her great-nieces and great-nephews, Adeline Osmond, Anthony Brannon, Bradley Brannon and Donavan Brannon.
