He was predeceased by: his parents, Stephen Kozlovich and Shirley Sumner Kozlovich; and his sister Gaye. He is survived by: his significant other Lisa Peterson; his significant other's children, Nicholas and Ryan Dimastrantonio; his grandsons, Trystan and Sydan; his siblings, Juli (Chris Libby), Lora (Tom Schaafsma), Owen Kozlovich and Sara (Thomas Russell); and his aunt Ella Sumner Sweetnam of Ontario. He is also survived by nephews, nieces and cousins.
