He is survived by: his wife Jennifer S Smith; his mother Kathleen Smith (Meagher) of Rockford, IL; his children, Kathleen Koontz (Kristopher) of Crown Point, IN and Richard Allen Smith (Melissa) of Nixa, MO; his grandchildren, Arianna, Angelina, Grayson and Gaige; and his siblings, Linda King of Rockford, IL, Joanne Williams of Rockford, IL, Carol Martin of North Andover, MA, James Smith of Colorado and Steven.
