He was predeceased by: his parents, Helen Chase (Ward) and Curtis Chase; and his brother Paul. He is survived by: his close friend Tina; his children, Lisa (Larry Foulkes) of Williamsburg and Jason (Crystal) of Medford; his wife Tina; his grandchildren, Mariah, Mercedes, Ryan, Gavin and Bella; and his sisters, Phyllis Ireland of Veazie and Zephyrhills. He is also survived by the numerous special friends.
