He is survived by: his sons, Tom and Thomas Allen Levensalor (Nichol); his brothers, Thomas Levensalor (Donna), Terry Levensalor (Joanne Tucker) and Toby Levensalor (Nicole); and his uncle Laurence Levensalor. He is also survived by his nieces and nephews.
