He was predeceased by: his grandparents, Robert, Betty Elliott, Harold and Mavis Blake; and his uncle Albert Elliott.
He is survived by: his parents, Dennis Blake and Barbara Blake of Freeman Twp; his brother Scott (Shari); his daughter Kaitlynn; his stepson Brett of Morganton, NC; his children, Berlynn, Brady, Beau and Henry; and his aunts and uncles, Ronald, Meridell Elliott, James, Kim Elliott, Cheryl and Lincoln Stone.
