He was predeceased by: his parent Richard M. Lewis Sr.; his former wife Christine Lewis; and his brother John E. Lewis. He is survived by: his mother Jacqueline "Jackie" Lewis; his husband Roger Provost of Ft; his brother Michael S. Lewis (Denise) of West Bridgewater; and his nephew John M. Lewis of West Bridgewater.
