He was predeceased by: his parents, Claire and Louis; his sister Peggy; and his former wife Elizabeth. He is survived by: his children, Joel and Judy Jolicoeur; his son-in-law Frank Nava; his nephew Tom Page; and his niece Michelle Roberts.

To honor Jack's memory, donations can be made to the Shriner's Hospital.
