He was predeceased by: his parents, Carolee Hurlburt (Cowan) and John Gailen Hurlburt. He is survived by: his girlfriend Kimberly Herrin of Dixmont; his sons, Jason and Kyle; his grandchildren, Vivian and Lincoln; his sister Robin Chase (Michael); his stepmothers, Rose Neal and Eleanor Hurlburt; his niece Katelyn Wright; his great-nephew Beau; and his stepsister Pamela Snowman (Tom).
