He was predeceased by: his parents, John Cislak and Mary Cislak (Lemanski); and his siblings, Ilona "Cookie" Moeckel and John Cislak of Massachusetts. He is survived by: his wife Marilyn Cislak (Holst) of Alexander; his children, Shaun Cislak of Massachusetts and Stacey Decesare of Maine; and his sister Maryanne (Leonard Frost) of Alexander.
