He was predeceased by: his parents, Wilfred Provencher and Blanche Provencher (Cook). He is survived by: his wife Joan Cook (Anderson) of Fayette; his children, Richard W. Cook, Jr. (Catherine) of Goffstown, NH, Robin D. Cook (Lisa Henchel) of Manchester, ME, Erin Heilig (Tom Ross) of Wilmington, DE and Joy Powell (Duncan Widdoes) of Spokane, WA; his stepsister Rita Cribbin (Michael) of Greene, ME; his grandchildren, Aaron, Ashley, Robin Jr. and Nathan; and his great grandchildren, Micheala, Abigail and Sullivan.
In lieu of flowers, the family requests that donations in Dick's memory be made to: the Cullen Fund at Penney Memorial United Baptist Church in Augusta, the Kennebec Valley Humane Society and the Maine Veterans Homes.
