He is survived by: his wife Loretta Mayforth; his children, Michael Mayforth (Shannon) and Megan Miner (Patrick); his grandchildren, Samantha Mayforth, Drew Mayforth, Ruby Miner and Jacob Miner; and his siblings, Albert Mayforth (Phyllis) and Molly Campbell (Chris).
