He was predeceased by: his parents, Theodore, Sr. Cook and Mary Cook; his wife Margaret "Ann" Roberts Cook; and his grandson Devin Miller. He is survived by: his children, Carole Eckard, Ted Cook III (Mary), John Cook (Cindy), Jim Cook (Diane), Susan Franks (Mickey), Patrick Cook, Steven Cook (Lori) and Daniel Cook (Connie); and his sister Betty. He is also survived by 11 grandchildren, 16 great-grandchildren; 2 great-great grandchildren.
