He was predeceased by: his parents, George Edward and Helen Smith (Barrett); his spouse Mary Kathleen "Kathy" Smith; and his siblings, Sonny and Betty.

He is survived by: his child Pamela Cook; his grandchildren, Brittany, Kayla, Jodi, Sabrina, Gracie and Jack; his great grandchildren, Hunter and Hektor; and his brothers, Butch, Danny, Toe, Eddie and Tim.
