He was predeceased by: his grandmother Norma Chadwick; and his great-grandparent Norman Simmons. He is survived by: his siblings, Eric, Gaxon, BrynEllari Crouse, Alan Sullivan and Patrick Sullivan; his great-grandparent Carol Simmons; his grandmother Lisa Crouse; his mother Dottie Burdsall; and his aunts and uncles, Jamie Kelly, Kristina Sten, Amanda Chadwick, Alecia Foote, Mickey Crouse, Anthony Chadwick and John Simmons. He is also survived by numerous cousins and extended family.
