He was predeceased by: his parents, Mark M. Duckworth and Mary Ellen Duckworth (Hamilton); his daughter Carrie Beitzel; and his brothers, Gary Bruce and Mark Samford Duckworth.

He is survived by: his fiancé Beverly Ann Beitzel; his grandchildren, Kaitlyn, Christopher, Courtney and Carrie; and his great grandchild Ares.
