He was predeceased by: his parents, David A. Carmack and Mary Carmack (Karaffa). He is survived by: his wife Shelby K. Carmack (Stokes); his sister Nancy I. Kelble (Harry) of North Port, FL; and his goddaughter Kari Lynn Toney (Mankey). He is also survived by many nieces and nephews.
