He was predeceased by: his father James Van Slyke. He is survived by: his wife Michele; his daughter Katie (Charlie); his brothers, Gene Van Slyke (Amy) and Scot (Joyce); and his grandchildren, Wyatt, Ellie, Brett, Luke and Chase. He is also survived by nieces, nephews, and cousins.
