He was predeceased by: his parents, Joseph Culotta and Anna Culotta; and his brothers, Joseph Culotta, Jr., John Culotta, Richard Culotta and Joseph Rodgers, Jr..

He is survived by: his close friend Jackie Zulauf; his children, Salvatore "Sammy" A. Culotta, Jr. (Stella) and Mary A. Johnson (Joe); his grandchildren, Christine, Michelle, Kimberly, Aaron and Amber; and also Leona Risher (Charles). He is also survived by 5 great grandchildren, nieces, nephews.
