He was predeceased by: his parents, James J. Mazingo and Nellie Lenore Cubbage Mazingo; and his siblings, Jeffrey Mazingo, Patsy Hoffman Koch, Roger Hoffman and Foster Hoffman.
He is survived by: his brothers, James O. Mazingo (Barbara) of Mesa, AZ, Dennis G. Mazingo (Ann) of Hagerstown, MD and Calvin Hoffman (Velda) of Berkeley Springs, WV.; and his grandson Jared Cubbage. He is also survived by many nieces and nephews.
