He is survived by: his mother Betty Middlekauff; his daughters, Brittney Haines and Belinda Pettis (Blue Pettis); and his sisters, Karen Grimes (Jeff), Linda Wallech, Debbie Jones (Mike) and Melissa Rhodes. He is also survived by six grandchildren, and nieces and nephews.
