He was predeceased by: his parents, Sheppard Smith and Mary Smith (McClean); his son Haywood Smith; and his sister Nancy McMillian. He is survived by: his wife Jeanne L. Litten; his children, Orlando Smith, Larry Smith, Joey Smith, Tonya Smith and Henry S. Smith, Jr.; and his brother George Lewis Smith. He is also survived by eleven grandchildren; and numerous great-grandchildren.

Memorial contributions may be made in Henry's name to the Pulmonary Fibrosis Foundation, 230 E. Ohio St., Suite 500, Chicago, Ill. 60611.
