She is survived by: her husband Dr. Leonard Jakubczak; her children, John Jakubczak (Holly), Jane Jakubczak (Tony Spinosa), Anne Edwards (Wells) and Lynn Cavanaugh (David); and her grandchildren, Shelby Samuels, Quinn, Connor Cavanaugh, Ben and Alex Jakubczak.
