He was predeceased by: his parents, William Slade, Sr. and Elaine Moats. He is survived by: his siblings, Lori Wilson (David), Bonnie Gadoua (Mike), William Slade, Jr. (Robin) and Scott Mahan (Angela); and his caregiver Dawn Robinson. He is also survived by extended family members and dear friends.
