He was predeceased by: his parents, John Hoffman and Carmella Hoffman; and his wife Karen Hoffman. He is survived by: his brother Michael (Sue Hoffman); his mother-in-law Deloris Phillips; and his sister-in-law Donna (Harvey Jacob).
