He was predeceased by: his mother Karen Petit. He is survived by: his wife Tambra Lavis; his children, Shawn Anthony Collins, Brian Anthony Cave, David Michael Lavis, Laciann Marie Lavis and Timothy James Ray Lavis; and his grandchildren, Bella, Brayden, Carson and Aurora. He is also survived by great father, who loved his children and grandchildren.
