He was predeceased by: his siblings, Carol Mauke (Hebert) of Webster and Joseph Hebert of Dudley; and his parents, Samuel Hoffman, Sr. and Mary Hoffman (Beaudette). He is survived by: his wife Lisa Hoffman (Derusha) of Greenfield; his daughters, Marie Tinsley (Jay Tinsley) of Southbridge and Carrie Morse (Hoffman) of Rhode Island; and his siblings, Truffly Hoffman of Dudley, Elizabert Davis (Hoffman) of North Carolina and Jeanne Germain (Hebert) of Webster. He is also survived by four grandchildren, one great-grandchild, several nieces and nephews.
